Dear CPC Scholarship Committee:

Our chapter wishes to apply for a chapter scholarship. Attached is an essay of the chapter's plans for the use of the funds and why our chapter should be selected as a scholarship recipient.

Our chapter acknowledges that we will be required to provide a report to the CPC, listing the distribution of the funds and our chapter's resulting successes.We understand that failure to do so will disqualify our chapter from applying for the scholarship in the next calendar year.

* Chapters must have paid their 2009 contribution to the CPC scholarship fund to apply for the CPC Chapter Scholarship and have attended a minimum of two CPC meetings in the past calendar year.
